COUNTY NEW RESIDENTIAL SUBSCRIPTION REQUEST FORM
If you wish to start residential
service at your home in our service area of Clay County, please provide
the information requested below and submit the form by clicking on the
submit block at the bottom of the form.
The charge for residential
service is billed quarterly in advance. New customers will be billed
a prorated charge for the remainder of the first quarter of service.
